This opportunity is located in Rensselaer, New York.
St. Peter's Health Partners is seeking a Board Certified Hospice and Palliative Medicine physician to join a respected and expanding system of caregivers. The selected candidate will possess excellent clinical, communication and time management skills, as well as a strong commitment to providing excellent team oriented care.
The Hospice Medical Director meets with the Interdisciplinary Group (nurses, social work, chaplains and volunteers) on a routine basis regarding each patient’s needs. They will also provide support and guidance to the patient’s primary care provider in the community, and makes visits to patients at home and in care facilities.
The Hospice practice is a not-for-profit organization serving eight counties in the Capital Region of Upstate New York through four regional offices. The Community Hospice is a non-profit 501(c)(3) organization, accredited by the Community Health Accreditation Program. The practice is part of St. Peter's Health Partners, and is privileged to work with every hospital in the region to provide hospice services. The team collaborates with 43 skilled nursing facilities and many assistive living/adult homes across the service area. The majority of patients are served where they prefer to be at end of life to maximize quality of life – their home. Over 4,000 patients are served annually, with an average daily census of 450 patients. The service area covers: Albany, Schenectady, Rensselaer, Saratoga, Columbia, Greene, Washington and Montgomery Counties.
The group is an innovative organization, directing a well-established Palliative Care Partners service line across the affiliated health system (inpatient, outpatient, and home-based). A thorough orientation and supportive work environment is provided to employees.
A strong preference will be shown to candidates with three to five years of direct experience in hospice and/or palliative care.
