- Hours: Clinic hours, 8am-4pm, Monday through Friday (Provider will work 4 days/week)
License/Certification:
Nurse Practitioner:
- Minimum: Registered Nurse Licensure from the state of Indiana.
- Nurse Practitioner certification.
- Basic Life Support (BLS) certification required.
Physician Assistant:
- Minimum: Physician Assistant Licensure from the State of Indiana.
- National Commission on Certification of Physician Assistants (NCCPA) certification.
