Practice goals are to improve the multiple transitions of care patients and families may experience from the hospital stay, to post-acute facility admission, to back home. About the positions
- Practice providers currently care for patients at up to 10 post-acute facilities
- Facility medical directorship opportunities are available
- Licensed practical nurses assist with patient list maintenance and chart preparation for visits
- A registered nurse navigator and licensed social worker are available for transitions-in-care-continuum patients
