Nurse Practitioner - Family Medicine

Mercy Health in Defiance, Ohio is seeking a compassionate and dedicated Advanced Practice Provider (Family Nurse Practitioner or Physician Assistant) to join our Diabetic Education team. This provider will play a key role in supporting and educating patients managing both Type 1 and Type 2 diabetes.

You’ll guide patients as they build confidence in managing their diabetes through lifestyle coaching, medication support, and regular monitoring.

Position Highlights

• Outpatient Schedule: Monday–Friday, 8:00 AM–5:00 PM
• No traditional call — occasional PerfectServe notifications for urgent matters
• Telehealth options including virtual visits and e-visits
• Typical patient volume: 15–22 patients per day
• Established patient base with the ability to absorb the panel of a departing provider
• Reliable CMA support on site

Clinical Responsibilities

• Conduct comprehensive health assessments and patient histories
• Develop individualized treatment plans and diabetes education programs
• Guide patients in glucose monitoring, lifestyle modifications, and medication adherence
• Order and interpret diagnostic studies as appropriate
• Collaborate with physicians and multidisciplinary care teams
• Manage MyChart messages, patient calls, and specialty referrals
• Engage in professional duties consistent with APC practice standards
