We are seeking a Physician Lead (Medical Pod Leader) for our Philadelphia, PA market—an exceptional opportunity for a physician who thrives at the intersection of clinical excellence, leadership, and compassionate patient care.
This role blends direct primary care with meaningful leadership responsibilities, guiding a team of Advanced Practice Providers (APPs) and clinical professionals who serve some of the region’s most vulnerable patients.
As the Medical Pod Leader, you will serve as the key clinical leader for a designated care team ("pod"), ensuring high-quality outcomes, strong team performance, and seamless collaboration across disciplines. Working in dyad partnership with the Market Manager, you will mentor providers, support clinical operations, and contribute to market-level initiatives—while also maintaining a small panel of your own patients.

Clinical Leadership & Mentorship
Serve as the collaborating and supervising physician for APPs and physicians within your pod
Mentor, coach, and clinically develop APPs through ride-alongs, case reviews, and structured feedback
Lead care team huddles and ensure proactive plans for high-risk or complex patients
Support social workers, community health workers, nurse navigators, care managers, and other team members
Quality, Documentation & Performance
Conduct routine chart reviews and provider panel assessments
Review pod-level clinical quality metrics and provider productivity; implement improvement strategies
Guide documentation practices, diagnosis accuracy, and chronic condition management
Sign orders and provide oversight in accordance with state regulations
Patient Care
Maintain a small panel of your own patients, delivering high-touch home-based primary care
Perform high-risk patient visits or joint visits with APPs as needed
Provide clinical expertise for complex or escalated cases
Operational & Organizational Collaboration
Partner with Market Leadership and Regional Medical Directors to drive pod-level performance
Participate in hiring, onboarding, compliance reviews, and performance improvement, in collaboration with HR
Engage in strategic initiatives that support market growth and clinical excellence
MD or DO with an active, unrestricted Pennsylvania license
Board certification in Internal Medicine, Family Medicine, or Geriatric Medicine (preferred)
Demonstrated experience leading, mentoring, or collaborating with APPs strongly preferred
Strong clinical acumen with experience managing medically complex adult patients
Commitment to relationship-centered, home-based primary care
