Our Jacksonville team delivers high-touch, comprehensive care directly in the patient’s home, ensuring they receive the support and clinical oversight they need to stay healthier and avoid preventable hospitalizations.

We are seeking a Nurse Practitioner to join our Jacksonville, FL medical group and play a key role in providing compassionate, relationship-centered primary care to our patients.

Role Highlights

In this position, you will serve as a primary care provider for a small panel of adult patients receiving in-home medical care. This role is ideal for a clinician who values meaningful patient interactions, clinical autonomy, and a proactive approach to managing chronic disease. 

In this role, you will:

• Conduct routine in-home visits, spending the time needed— up to 60 minutes—to deliver thorough, patient-centered care - We provide a company car!

• Focus on preventive, proactive medical management to reduce unnecessary ED visits and hospital admissions

• Collaborate closely with an interdisciplinary team, including physicians, medical assistants, care managers, social workers

• Build long-term relationships with patients and families, helping them navigate complex health needs

• Enjoy a balanced schedule with telephonic only back-up call

Qualifications

• Active Florida Nurse Practitioner license

• National board certification AANP or ANCC

• DEA registration (or willingness to obtain)

• Valid driver’s license and reliable personal transportation

• Experience with geriatric or complex chronic care populations preferred, but not required
