Were seeking a passionate and driven Physician Assistant/Nurse Practitioner to join our Geriatrics team in Grand Rapids, Michigan. This is a dual role supporting both the Geriatrics Home-Based Primary Care (HBPC) program and the Geriatrics Primary Care Clinic. This is an exciting opportunity to make a meaningful impact on the health of our community while working in a supportive, mission-driven environment.

Opportunity Details

• Position type: Full-Time (1.0 FTE)
• Location: Grand Rapids, MI
• Specialty: Geriatrics Home-Based Primary Care (HBPC) program and the Geriatrics Primary Care Clinic
• Schedule: Monday to Friday, 8am to 5pm
• Call requirements: Shared between the providers on team
• Role requirements: The position requires strong clinical judgement, exceptional communication skills, and the flexibility to transition between home-based care and clinic-based care to meet the needs of geriatric patients with complex medical, functional, and social needs.

Requirements

• Provider experience preferred, open to new graduates with strong nursing experience
• APPs: Board certified in specialty prior to employment
• Active Michigan medical license (or ability to obtain)
• DEA license
• ACLS/BLS certification (must be maintained)

Duties & Responsibilities

A typical day may include:

• Providing direct patient care
• Collaborating with multidisciplinary teams
• Participating in teaching and mentoring
• Documenting in Epic EMR
• Engaging in quality improvement initiatives
• Supporting community health efforts
