Job Description

Monitors and tracks all patients who have been diagnosed with heart valve disease specifically but not limited to:

  • Maintain list of patients for aortic balloon Valvuloplasty, Mitral stenosis, pre TAVR patients, and patients with mitral clip.
  • Facilitate tests for patients
  • Prepare patient office records for presentation at valve clinic meeting (cath films, MRA, carotids, TEE, Lab Tests)
  • Post Procedure follow up scheduling
  • Gain familiarity with echo criteria and valve protocols and notify physician when questions arise.
  • Identifies patients in need of ongoing diagnostic interventions and related services; and facilitates initial and follow up encounters in multidisciplinary clinic.
  • Provides input to the Valve Clinic Advisory Committees in the development of quality programs that include tracking information, care conference presentations, outcome measures and patient satisfaction indices.
  • Develops and manages monitoring systems to help meet quality initiatives.
  • Organizes and actively participates in a regular multidisciplinary valve clinic.
  • Demonstrates sensitivity to the needs of patients, families, staff, and physicians. Responds to concerns, questions, and other feedback in timely and thoughtful manner.
  • Coordination of information and patient transfer to other hospitals that have availability to do valve procedures.
  • Works with patients, providers and caregivers to identify possible barriers to care and systematically helps to break down barriers (i.e. Financial, transportation, language, etc.).
  • Facilitates as necessary timeliness of appointments; works continuously to reduce the intervals between time of abnormal results, diagnosis and treatment.
  • Develops and maintains relationships with providers, internal service delivery departments, other caregivers and patients in order to coordinate services within the organization, at outside facilities and within the community.
  • Assists patients in accessing support and follow-up services.
  • Maintains contact with patients throughout the continuum of care; meets patients at their doctors appointments as necessary; addresses questions and concerns.
  • With the Medical Directors and leadership team, assists with the development and implementation of goals and objectives (including staff education) for the Valve clinic.
  • Maintain list of patients with valvular heart disease referred to Cardiac Surgery and the Multidisciplinary valve clinic.
  • Refine and maintain the ECHO AI reports as a quality initiative to ensure adequate capture of all patients with valvular heart disease.

 

Key Responsibilities:

  • Facilitating patient evaluation, procedural planning, data management, and quality improvement initiatives.
  • Serving as the primary liaison between patients and the Structural Heart team to ensure timely communication and care coordination.
  • Participating in patient and family education, setting expectations, and answering questions related to the procedure and recovery process.
  • Ensuring timely and accurate data entry into national registries and monitoring program metrics and KPIs.
  • Ensuring compliance with CMS National Coverage Determination (NCD) requirements and institutional policies.
  • Participating in community outreach and physician education to promote awareness and appropriate referrals to the Structural Heart program.
  • Developing and maintaining relationships with internal and external referring physicians and developing a program database

 

Qualifications:

  • Masters Degree
  • National Board Certification as an Acute Care Nurse Practitioner or Physician Assistant
  • BLS certification
  • ACLS certification
