Required Skills:

  • Active Iowa Medical License and current NP certification
  • Proficiency in providing comprehensive in-home and telehealth assessments for both pediatric and adult patients
  • Strong clinical judgment to identify urgent issues and coordinate appropriate care
  • Excellent communication skills to effectively engage with patients and healthcare teams
  • Ability to perform risk assessments, chronic condition reviews, and A1C testing
  • Reliable time management to handle 5-6 assessments daily with adherence to schedule
  • Comfort with technology and electronic health records (EHR) systems

Nice to Have Skills:
  • Prior experience in home health or community-based practice
  • Familiarity with telehealth platforms and remote patient monitoring
  • Bilingual abilities to serve diverse patient populations
  • Knowledge of Medicare and Medicaid regulations related to home health services

Preferred Education and Experience:
  • Master's degree in Nursing (MSN) or higher
  • Minimum of 2 years of family practice or primary care experience
  • Experience with telehealth and home health assessments highly valued

Other Requirements:
  • Willingness to travel within targeted counties (Linn, Polk, Black Hawk, Scott); mileage reimbursed at IRS standard rate
  • Ability to commit to full-time (40 hours/week) or part-time schedules, with schedules released 2-3 weeks in advance
  • Enrolled and credentialed in Medicard system
  • Ability to complete required orientation and ongoing education during work hours
