Remote admissions across multiple Wisconsin regions
Location: St. Lukes Professional Office Building/ Zilber
Lead initial patient prognosis reviews, medication and treatment assessments, and goals of care discussions, collaborating closely with multiple teams at the Zilber hospice facility.

Annual Salary Base Plus: $276,143-$276,143

TRAINING AND/OR EXPERIENCE REQUIRED
Preferred candidates have hospice or palliative experience, with completion of an HPM fellowship not required
