Banner Health, one of the nation's premier non-profit health systems has an excellent opportunity for a Family Nurse Practitioner or Physician Assistant to join our well-established Primary Care Team in Greeley, Colorado! If you are looking for an opportunity to work in an outpatient setting while working alongside collaborative team members with flexible scheduling, then look no further!

Our Advanced Practice Provider will have their own patient panel and clinical responsibilities to include all ages, wellness, and chronic care disease management, as well as specialized procedures according to candidate's interest.

Minimum Requirements:

• FNP designation or PA-C certification with CO license eligibility
• DEA with prescriptive authority required
• Patient-focused, friendly, and willing to collaborate in a team environment

Position Details:

• Outpatient Primary Care clinic with Pharmacy, Lab and Imaging on site
• Clinic hours: Monday-Friday, 7:00 AM-5:30 PM; Saturdays 8 AM-12 PM
• Neighborhood clinic treating all ages
• Flexible 4-day work week (10-hour shifts) with occasional Saturday coverage
• 36 hours of patient contact per week; 4 hours dedicated admin time
• Minimal call coverage - phone consultation only
• Medical Assistants, Front Office, Referrals and leadership support
• Support of an integrated system including specialists, urgent care, and more
• Eligible for Public Service Loan Forgiveness program

Compensation & Benefits:

• Salary range: $120,000-$145,000 annually (dependent on experience)
• $5,000 sign-on bonus
