Primary Care Darby, part of Mercy Fitzgerald Hospital, is seeking a Primary Care- Nurse Practitioner to support continued primary care expansion and access initiatives across the Pennsylvania market. 

The Practice

• Established practice  
• Opportunity to build a robust patient panel 
• Organizational support to drive practice growth and community engagement

• Strong referral network within Trinity Health Mid-Atlantic 

The Opportunity

• Provide comprehensive primary care for patients across the lifespan 
• Assist with acute and chronic diagnosis and management of conditions
• Emphasize preventive care and population health management 
• Collaborate with interdisciplinary teams to optimize patient outcomes 
• Maintain timely and accurate documentation 
• Participate in quality and performance improvement initiatives 

Schedule & Call 

• Full-time schedule (36 patient-facing hours)
• Flexible scheduling options available 

Qualifications 

• Nurse Practitioner license from an accredited institution 
• Eligible for Pennsylvania license
• Current DEA license (or ability to obtain prior to start date)
• Current BLS certification required
• Commitment to high-quality, patient-centered care
