• Coverage Type: Call only
• Schedule Type: Call only
• Call Type: Beeper
• Dates of Need: Varies, 1:3 call rotation
• Shift Times: Thursday at 7AM MST to Monday at 7AM MST
• Provider Minimum Shifts Required: 1:3 call rotation every month ongoing
• Guaranteed Hours per Day: N/A, call
• Hours Worked for OT to Apply: N/A, call
• Rates to be filled out on bid: 24 Hour Beeper Call: Orientation, 24 Hour Call, Call-Back

Credentialing Requirements:

• Current CV (must be uploaded to bid)
• License: Active or IMLC
• DEA Required: Yes
• Boards: BC only
• Skills/Certifications: N/A
• Other: N/A

Additional Details:
Provider will use our secure paging service Thursday at 7AM MST - Monday at 7AM MST during the 1:3 call rotation.
