PATHS is seeking a Family Nurse Practitioner or Physician Assistant to provide high-quality, patient-centered primary care in our Danville clinic. You'll join an experienced, collaborative care team practicing evidence-based medicine within a Patient-Centered Medical Home (PCMH), caring for patients across the lifespan while making a meaningful impact in the community.

Schedule
- Full-time (40 hours/week)

What You'll Do
As a member of the PATHS care team, you'll provide comprehensive primary care while building long-term relationships with patients and families.
Responsibilities include:
- Perform comprehensive patient evaluations and physical examinations
- Diagnose and manage acute and chronic medical conditions
- Develop individualized treatment plans
- Order and interpret diagnostic testing
- Prescribe medications and manage refills
- Coordinate specialty referrals and follow-up care
- Educate patients on disease prevention and healthy lifestyles
- Review laboratory and imaging results with patients
- Maintain timely documentation within eClinicalWorks
- Participate in quality improvement initiatives and provider meetings
Qualifications
Required
- Licensed Family Nurse Practitioner (FNP) or Physician Assistant (PA) in Virginia
- Current DEA registration
- Eligible to bill Medicare, Medicaid, and commercial insurance
Preferred
- Board Certification
- Experience practicing in an FQHC
- Experience with eClinicalWorks
