Our value-based care model allows physicians and APPs to focus on quality outcomes and meaningful patient relationships rather than high patient volume. Providers visit medically complex patients in their homes and work alongside an interdisciplinary team that helps coordinate services, manage chronic conditions, and support patients between visits.

Providers typically see 5–9 patients per day, allowing time for thoughtful visits, care coordination, and strong relationships with patients and families. Patients come through established health plan partnerships, so there is no patient acquisition required—and no hospital rounding.

The Opportunity

HarmonyCares is expanding into the Lehigh Valley and South Central Pennsylvania and is seeking a Physician Lead (Medical Pod Leader) to help launch and grow our home-based primary care practice across the region.

Each provider will care for a geographically clustered patient panel within their local area, minimizing unnecessary travel while allowing the broader team to serve patients throughout multiple counties.

This role combines direct patient care with meaningful clinical leadership. With up to 50% of your time dedicated to leadership responsibilities, you will help build and guide the local practice, mentor and supervise a team of Advanced Practice Providers, and maintain a small patient panel of your own.

Qualifications

  • MD or DO with an active Pennsylvania medical license
  • Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred
  • Experience collaborating with or supervising Nurse Practitioners or Physician Assistants strongly preferred
  • Strong clinical experience caring for medically complex adult patients
  • Interest in leadership, value-based care, and relationship-centered medicine
