What You’ll Do
- Provide outpatient medical care for adult patients over 55, primarily within a PACE program setting
- Conduct in-home visits to assisted living, SNFs, or private residences, often with a nurse partner
- Collaborate with an interdisciplinary team to coordinate patient care
- Manage patient histories, perform examinations, and develop treatment plans
- Document clinical notes efficiently using electronic medical records (EMR)
- Ensure compliance with licensing and certification requirements
What You Need
Required: - Active medical license in Oregon
- DEA certification
- BLS certification
- Geriatric care experience preferred
- Ability to work independently with excellent communication skills
Preferred: - Prior outpatient or community-based care experience
- Collaborative team player with strong organizational skills
