Position Summary
The VA Northern California Health Care System is seeking an experienced and mission?driven Community Living Center (CLC) Medical Director to lead high?quality, Veteran?centered care at the Martinez VA Medical Center. This physician leader will oversee clinical operations, quality of care, and interdisciplinary collaboration within the Community Living Center, supporting Veterans requiring long?term care, short?stay rehabilitation, hospice, and palliative services.
This role offers a unique opportunity to combine clinical excellence, leadership, and service in a federal healthcare system dedicated to those who have served.
Key Responsibilities
- Provide clinical leadership and oversight for the Community Living Center, ensuring safe, high?quality, evidence?based care.
- Lead and collaborate with an interdisciplinary team including nursing, rehabilitation, social work, pharmacy, and mental health.
- Ensure compliance with VHA policies, Joint Commission standards, and CMS long?term care regulations.
- Oversee quality improvement initiatives, performance metrics, and patient safety programs.
- Support Veteran?centered care models, including hospice, palliative care, dementia care, and restorative services.
- Participate in medical staff leadership, peer review, credentialing, and performance management.
- Serve as a clinical resource and mentor for physicians and advanced practice providers within the CLC.
- Coordinate care transitions with acute care, outpatient services, and community partners.
Qualifications
Basic Requirements
- MD or DO from an accredited medical school.
- Board certification or board eligibility in Internal Medicine, Family Medicine, Geriatrics, or related specialty.
- Unrestricted U.S. medical license (any U.S. state or territory).
- Eligible for VA clinical privileges.
Preferred Qualifications
- Experience in long?term care, skilled nursing, geriatrics, hospice, or palliative care.
- Prior medical director or physician leadership experience.
- Strong interest in quality improvement, team?based care, and Veteran advocacy.
