Nurse Practitioner - Physical Medicine & Rehabilitation

We are a growing, physician-owned medical group expanding in Palm Desert, CA seeking an experienced Nurse Practitioner or Physician Assistant to join our team in a Skilled Nursing Facility (SNF) setting. This role focuses on post-acute and rehabilitative care, including orthopedic recovery, stroke management, and pain management, working alongside a collaborative lead provider. 

Key Responsibilities: 

• Provide comprehensive medical care to SNF patients, including assessments, diagnosis, and treatment planning 

• Manage patients with orthopedic rehab needs, post-stroke care, and chronic pain conditions 

• Ensure accurate and timely documentation in EMR 

Schedule & Setting: 

• Monday through Friday 

• Part-Time 

• No nights, no call, no weekends 

• “Round and go” model for strong work-life balance 

• Skilled Nursing Facility setting
